1. What Should I Bring to My First Appointment?
  • Identification: Bring a government-issued ID, like a driver’s license or passport.
  • Insurance Information: Have your insurance card and any relevant documentation.
  • Medical Records: If applicable, include previous test results, prescriptions, and a list of medications you’re currently taking.
  • Payment Method: Be prepared with a credit/debit card or cash for copayments.
  • Proof of Income: We accept the following forms of proof of income, which we will need to apply our sliding scale discounts:
    • Most recent tax return or tax transcript; 
    • 1099 or W-2 forms; 
    • Paycheck stub or other documentation from employer data within the preceding three months (no notarization required); 
    • WIC approval letter or other enrollment verification; 
    • SNAP/Food Stamps approval letter or other enrollment verification; 
    • Unemployment benefits approval letter or other enrollment verification; 
    • Bank account statements reflecting direct deposits by Social Security and/or employer(s); 
    • Social Security Benefit Verification letter (e.g., obtained via the process described at https://www.ssa.gov/pubs/EN-05-10552.pdf); 
    • Eligibility notices from the federal marketplace (www.healthcare.gov) within the previous 12 months that report an applicant’s household income;  
    • Eligibility notices from any state agency (e.g., TennCare, Department of Human Services, etc.) within the previous 12 months that report an applicant’s household income; 
    • Patient’s written attestation to having one or more children aged 1-18 enrolled in TennCare (which, by definition places the household under 138% of the federal poverty level); 
    • Patient’s written attestation to receiving benefits from the CoverRx program (which, by definition places the household at or below 100% of the federal poverty level) and provides a photocopy of his or her CoverRx card (or other verification of enrollment); and 
    • Patient’s written attestation to receiving benefits from the Supplemental Nutrition Assistance Program (SNAP, formerly Food Stamps) and provides a photocopy of his or her electronic benefits transfer (EBT) card. 
  • List Your Concerns: Jot down any symptoms, questions, or matters you want to discuss with your doctor.
  • Medication List: Note all medications, including dosages and schedules.
  • Review Health History: Be ready to discuss your medical background and family history.
  • Insurance: If you have coverage, know your copayment and deductible obligations.
  • Out-of-Pocket: Understand the full cost of services in the absence of insurance.
